帕金森病患者面孔情绪认知特点及其影响因素研究A study on characteristics and correlative factors of facial emotion recognition in Parkinson disease

摘要(Abstract):
目的探讨帕金森病患者面孔情绪认知的特点、其与执行功能的关系及其影响因素。方法采用24例表达高兴、悲伤、恐惧、愤怒情绪的人物面孔黑白照片对24例PD患者和24名正常对照进行测试。两组均采用一个简化了的连线测验进行执行功能检查。所有患者应用Hoehn-Yahr分期、统一帕金森病评定量表第三部分(UPDRSⅢ)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)及左旋多巴等效剂量(LDE)等对临床特点进行评定。对PD组与对照组比较差异有统计学意义的识别面孔情绪得分,与年龄、病程、受教育时间、MMSE评分、LDE、H-Y分期、UPDRSⅢ评分、HAMA、HAMD评分之间进行相关性分析;并对PD组连线时间与各面孔情绪得分之间进行相关性分析。结果 PD患者的连线时间〔(51.58±33.14)s〕较对照组〔(34.46±19.64)s〕明显延长(t=2.178,P=0.036),连线错误次数(1.29±2.16)较对照组(0.17±0.64)明显增加(t=2.45,P=0.021)。PD组识别愤怒面孔得分(3.67±1.43)较对照组(4.71±1.20)明显减低(t=-2.731,P=0.009),识别高兴、悲伤和恐惧面孔得分与对照组比较差异无统计学意义(均P>0.05)。PD组识别愤怒面孔得分与年龄、病程、受教育时间、MMSE评分、UPDRSⅢ评分、H-Y分期、HAMA评分、HAMD评分及LDE均无相关性(均P>0.05)。连线时间与PD组识别恐惧面孔得分(r=-0.504,P=0.012)、识别愤怒面孔得分(r=-0.424,P=0.039)存在相关性。结论 PD患者的面孔情绪认知受损,可能与执行功能障碍有关。
